Probably fglrx-installer.
However I am not using fglrx , I am using xserver-xorg-video-ati and xserver-xorg-video-amdgpu.
This happened when I installed kernel 4.3.0-040300rc4 in Ubuntu Wily Werewolf.

When I got the crash fglrx-core was installed. I removed fglrx-core,
removed the Linux 4.3 RC4 kernel and installed it again. This time there
was no crash.

15.10 isn't supported anymore, and fglrx is not available in 16.04 and should get removed on upgrade

kernel 4.3 is not in any ubuntu release, so building the driver against it isn't supported either

I had https://bugs.launchpad.net/ubuntu/+source/fglrx-installer-updates/+bug/1690496 when inserted a new amd rx480 card and followed a guide at http://spacience.blogspot.ru/2015/11/gpu-mining-in-ethereum-1404-from-scratch.html at the step: sudo apt-get install fglrx-updates

if the xenial hwe stack is used then fglrx has no chance of working anyway since it won't load on xserver 1.18

I got this bug while plugging in my HDMI cable, screen was hanging (super lag, it seems like the GPU rendering error). It works fine with VGA interface.

This is a very old bug.
As far as I know support of fglrx is cancelled and replaced with amdgpu-pro.
I'm using amdgpu now and have no problems.
Additional drivers don't offers on the Ubuntu 17.10 beta I'm using now.
