Hence, as per your logs, you were using the 304.x nvidia driver when you first reported. As per nvidia.com, the latest available is 340.93, which is available in Wily as per https://launchpad.net/ubuntu/+source/nvidia-graphics-drivers-340 . Hence, would testing a newer version of the driver change anything?
