"PM Resume from disk failed " is the only error I saw in syslog, so I look in /etc/initramfs-tools/conf.d/ and surprised to find that it's an empty directory.
Well I make /etc/initramfs-tools/conf.d/resume with RESUME=/dev/sda4 and then do sudo update-initramfs -u ( /dev/sda4 is my swap partition)
then hibernate works - Thanks to whoever pointed it out :D
