This bug affects all operating systems. I have seen it on numerous versions of Ubuntu (9.04, 9.10, 10.04) as well as Windows Vista/7. Other users have also reported seeing it on Mac OS X on launchpad in this bug (of which 641300 mentioned above is a duplicate):

I have not discovered a specific way to reproduce it - it occurs very intermittently but when it does happen it is extremely annoying. 27 other people have also reported suffering from this defect (not including additional people who may have reported duplicate issues) on Launchpad.
