 I got this problem solved for me

 I also got into same problem after I upgraded from 8.04 to 8.10 and it persisted even after I moved to 9.04. While doing checks after the upgrade I noticed that I was running old kernel 2.6.22.14 that came as part of my 8.04 install. I retained my old /boot/grub/menu.lst that has reference to this old kernel. I forgot to change after the upgrades. When I changed it to the latest 2.6.28.16, I stopped seeing this problem. Hence for me the root cause is wrong kernel. see if this helps you all.
