I've also found that whenever i change the hdmi resolution back to one of the resolutions that the tv originally reported, the audio over hdmi begins to work again.
if i again change to the the tv native resolution (set with xrandr), the audio over hdmi stops again.

while the audio is not working, in the 'PulseAudio volume control' the output bars are still moving as thought sound is actually playing.
