I wanted to let you know that I solved this problem yesterday.  It's a documentation issue as far as people like me are concerned.  I thought that the Vista OS may have done something to the USB stick when writing a file to it.  So I rebooted under Vista and tried reformatting the "disk".  That didn't work.  Then I looked for something similar in Ubuntu without much luck.  But I did see a utility to create a boot image on a USB stick and opened it.  It had the format option I was looking for, and I used it - just the format - I didn't go on to create the bootable USB.  That worked.

However, it is still unclear what exactly is going wrong. Could you please provide some more details about the cause of the bug and the environment in which it was triggered?
Where you copying the files from your hard disk, or from another device or a network location, like bug #496231?
