However, it is still unclear what exactly is going wrong. Could you please provide some more details about the cause of the bug and the environment in which it was triggered?
Where you copying the files from your hard disk, or from another device or a network location, like bug #496231?

I wanted to let you know that I solved this problem yesterday.  It's a documentation issue as far as people like me are concerned.  I thought that the Vista OS may have done something to the USB stick when writing a file to it.  So I rebooted under Vista and tried reformatting the "disk".  That didn't work.  Then I looked for something similar in Ubuntu without much luck.  But I did see a utility to create a boot image on a USB stick and opened it.  It had the format option I was looking for, and I used it - just the format - I didn't go on to create the bootable USB.  That worked.

Great to hear you found a way to solve this issue!
I reckon this was an issue with the way the factory formatted your stick. In that case it wouldn't be a bug in Ubuntu -- although it would be worth providing support for it if it occurs often and when Windows Vista is able to support it.

Could you please reformat the USB device in Vista as well and check if it is still usable with Ubuntu?

I'm marking this bug Invalid after feedback from the reporter via a personal email stating that after formatting the stick on Windows Vista it still works.
