I just checked the CD images as of 2016-01-11 and 2016-01-25 and found the following on xenial-server-s390x.iso 25-Jan-2016 07:01 581M Server install image for IBM System z computers (standard download):

/boot/ubuntu.ins is OK
/ubuntu.ins is missing (/d390.ins was not replaced, but deleted)

Please create an /ubuntu.ins with proper paths to /boot/, too.
