Unfortunately, that didn't cut it for me.
[root@redmine-test redminelocalavatars]# patch -p0<rm21x-avatar.patch
patching file app/views/users/_avatar.erb.html
patching file lib/my_controller_patch.rb

Results: still 404 (http://pastebin.com/tmP2KsUU)

Fixes with erb.html -> html.erb are nice pick, regardless. Maybe there is something else?
