I can confirm, that the problem exists in inkscape 0.47, Mac OS X 10.4.11. I have it on both: X11 and aqua, which may help to track it down. I do attach a screenshot of the OP's pdf, displayed in both: preview.app and inkscape-aqua.app

It does not affect jpg files
