1. Add sign offs

2. Fixes an issue where "ack_staff" was not successfully applied when invoking the copy alerts dialog from the catalog Holdings View.

3. Related to testing this bug, I noticed the Holdings View in the catalog copy alerts count column included copy alerts that had already been ACK'ed. Patch included to avoid counting ACK'ed alerts.
