OOPS: requesting an upstream fix doesn't check input for duplicates

Bug Description

If you click "Request fix..." on a bug page and type in the name of a product that already has a task for that bug, you get an OOPS. Example: OOPS-53A549. This is the product equivalent of bug 4845.

Fixing this is a matter of adding input validation.
