display/pc crashes (static looking display) after waking from suspend

Bug Description
The display/system, as stated, freezes/crashes after waking from suspend.Looks a bit like an old static tv when I open the laptop. The same static did appear once when shutting down, at the end of the power down sequence.
UPDATE: Just found out this also just happens on logging out of a user. Goes into a static screen that needs a force restart to get out of.
This issue only occurs on this new kernel my system has been updated to, I think I submitted the report on my system after restarting.
The previous kernel that was OEM never had this issue, and is solved by utilizing the grub menu at boot and selecting the older "OEM" kernel.

System: Lenovo Thinkpad T14 Gen 1 on Ubuntu 20.04.3 LTS (latest updates with fresh install)
Kernel(with related issue): 5.13.0-28-generic
I could not figure out how to report the kernel specifically, but please point me in the right direction of where I would need to go to submit a bug report for a kernel issue such as this that affects the display/system.
