I'd say the Update Manager is the most obvious example of this, happens every time. It pops up when starting an upgrade on top of all other windows but doesn't have focus. It doesn't help to click on one of the windows behind it to bring that one to front, Update Manager still lies on top without focus. To get rid of it you first have to click on it to give it focus and then on one of the other windows to bring Update Manager to back again.
