since:
manny said the app button doesn't have to a small green icon, it can be a title
manny said the app button / title doesn't need to be clicked, it can be moused over

how about:
[x] in maximized windows, when I mouse over the title bar, the menu bar appears where the title bar was
[x] in non-maximized windows, when I mouse over the title bar, the menu bar appears below the title bar
