I built one more test kernel with only commit:
395a2076b406 cifs: connect to servername instead of IP for IPC$ share

The test kernel can also be downloaded from:
http://kernel.ubuntu.com/~jsalisbury/lp1792580

Can you test this kernel and see if it resolves this bug?
