I have been facing this problem for a long time now and it still doesn't seem to get fixed. To me it happens most often on multimonitor using HDMI, but also with a single monitor or multimonitor using VGA.
Using a Dell Studio XPS 1340 laptop with:
Ubuntu 12.04
Kernel: Linux 3.2.0-57-generic x86_64 GNU/Linux
CPU: Intel(R) Core(TM)2 Duo CPU P8700 @ 2.53GHz
GPU - NVIDIA SLI:
nvidia-smi -L
GPU 0: GeForce G210M (UUID: GPU-1b0674e3-9c44-6152-8b35-14ac258e89df)
GPU 1: GeForce 9400M G (UUID: GPU-0c72438f-4b81-5d92-cf00-974a003c6fc5)
Driver Version: NVIDIA-Linux-x86_64-331.20 - dowloaded from NVIDIA site, but getting the same problem with the ones in the Ubuntu repository
I've been losing a lot of time trying to fix it by reinstalling NVIDIA drivers. It caused me to lose a lot of work by freezing my computer randomly.
