Firstly, it looks like the amdgpu driver is repeatedly re-detecting your Samsung monitor. This might be a bug in the amdgpu driver, but might also be a faulty monitor connection.
Secondly, your "CM Storm Quickfire Rapid i" is being detected as only recently plugged in. So that would cause the screen to wake up.
Please try:
* A different monitor cable.
* A different keyboard, or just leaving the keyboard unplugged for a while.
And tell us if those stop the screen coming back on.
