alx (iamalx) hit the nail in the head for me. My wireless mouse had been having this random issue ever since Ubuntu 10.04 (now on Linux 13 Maya) running 3.20.0-36 and xserver-xorg 1:7.6+12ubuntu2

I have a Logitech MX100 wireless mouse.

I hope this helps some other poor soul looking for answers.
