The allegedly "FATAL" errors are actually ignored, so this shouldn't be what's causing it to fail. It would be useful to know what the hard-to-read errors on the failing install actually are; is there any possibility of deciphering them?
This can't be a debian-installer bug, since d-i isn't used on the live CD. I'm guessing usplash for lack of better information, but it could be a number of things.
Debian Etch doesn't ship a live CD so isn't much of a basis for comparison here. It would still be useful for you to test Ubuntu 7.04 (just a quick test-boot of the live CD) if at all possible.
