This is affecting me on a fresh install of 12.04 with updates.

I immediately ran all updates following installation, so I'm unable to report if the behavior existed prior to updates.

It seems to affect a variety of applications with no discernible consistency - I've specifically seen it in Calibre, google-chrome, and VirtualBox. As described above, there are varying combinations of app icons being missing from the alt-tab switcher, the dock, or both.
