I've created a video that illustrates what's going on here. I open LibreOffice Writer, and open a file. Note how the file is loaded, but the title bar of LibreOffice Writer still says 'Untitled 1 - LIbreOffice Writer' rather than changing to say 'testdoc1.odt - LibreOffice Writer' as it should. Sorry about the video quality, but using one of the desktop recording applications produces even worse results.
