I very much suspect that bug 438065 was the culprit for most, if not all, of these problems. Unfortunately I only learned about that one yesterday, so I couldn't mark it as a duplicate right away and had to torture you with all these debugging steps.
But I'm glad it's resolved now, thanks for your report!
