After some tests I was able to reproduce the issue on Maverick and with upstream Qemu (08218b3527301760393b0b4ec732fcdfb7ff6cda).
Also tried stressing the disk with fsstress and dd for many hours, but couldn't reproduce the issue. The easier way to reproduce it is with dpkg, but hard to find the exact cause.

My cmd line:
./rootstock --fqdn beagleboard --login ubuntu --password temppwd --seed ubuntu-netbook -i 3G --dist lucid --serial ttyS2 --components "main universe multiverse" --keepimage
The only thing I changed was the path of qemu-system-arm inside rootstock, to get the upstream one.
