This problem appears to be predicated on the auto-save combined with changing the picture. It appeared to be more noticeable when saving in/restoring from a .docx document.
Because it is related to auto-save, which by default is 15 minutes, then it will be as serious issue with long documents. it is a significant bug as the author will have little or no indication it has happened. Sending a document with missing drawings could affect the author's credibility.
