Comment 6 for bug 157249

If you open the file listed below and make a change to the document while having auto-save turned on the image on page 4 will disappear and be replaced with 'read-error' once the auto-save occurs. This seems to be an ooo-build issue. It might be related to the other issue I reported that showed read-error but I am not certain.
