I have determined that (on my system) disabling the Nvidia video driver fixes this problem.
When using the Nvidia video driver the following is found in the Xorg.0.log file:

Installed versions are:
nvidia-96 96.43.14-0ubuntu13
xserver-xorg-video-nv 1.2.1.15-1ubuntu3
