It happened to me with a freshly installed Ubuntu version. It worked fine with 14.10 and 14.04.

So all you need to do is to install Ubuntu 15.04 and configure a modem connection using the network manager applet.

I also will try to reproduce this error from scratch. Maybe also an USB stick version will do the job.
