Thank you for taking the time to report this bug and helping to make Ubuntu better.

How did you install mysql? From the dpkg history log I did not see a reference to install mysql. From the logs it also appears some of the tables do not exist. Please try running mysql_upgrade, but I am more curious as to how mysql got onto the system in the first place causing these tables to not exist.

Since there isn't enough information in your report to differentiate between a local configuration problem and a bug in Ubuntu, I'm marking this bug as Incomplete.
