Restarting the applet does nothing; if the logic got stuck somewhere, you much better killall -9 modem-manager and/or restart NetworkManager directly; that should be equivalent to a reboot.
Also, please attach ModemManager debug logs as per:

And please test with latest package (from the PPA) as suggested by Marius, we fixed some critical issue in Huawei modems a couple of weeks ago.
