Since this issue has gotten some attention lately I've conducted a few more tests. It seems that my first test was erroneous as the keyboard and mouse works if plugged directly into the computer, but only if the KVM is not plugged in.
This is the "fault-table" I came up with (the cpu was cold started for each configuration):
Working | Configuration

No | M&K directly and KVM to another USB port
Yes | M&K directly and an USB 2.0 hub
Yes | M&K in USB 2.0 hub
No | M&K in USB 2.0 hub and KVM to another USB port

My KVM is reported to be a USB 1.0 device, which might be the underlaying cause. It's a shame I don't have a USB 1.0 hub to back this up.
