This has happened to me many times too. Most recently today with Ubuntu 11.04 (Linux phoebe 2.6.38-11-generic #50-Ubuntu SMP Mon Sep 12 21:17:25 UTC 2011 x86_64 x86_64 x86_64 GNU/Linux)

All my USB ports stop working and I need to cold boot to get them back.

I _was_ running my mouse through the hub in my monitor. Now I've removed it from the hub, and will test with nothing running off the hub. Failing that, I will unplug the hub. It would be a shame, because the hub worked perfectly for years on older machines with earlier Ubuntu versions.
