I had finally time to check this a bit and looks like reason was few broken files.
I ran fsck which noticed a few broken files. Then I ran fsck with autorepair for that device/mp3-player and problems seems to be vanished.
So, I suggest that gui tool should do this automagically (with some confirmation) behind scenes.
