LFE (the subwoofer mixer control to which I'm referring) is wired incorrectly on many machines. On some machines, LFE is not correctly implemented with filtering. So, PulseAudio cannot assume that LFE works correctly on every machine. It is not surprising that the configuration option of enabling LFE remixing works on some machines and completely breaks on others.

If the volume is being changed after a stream ends, that isn't necessarily a PulseAudio issue. In fact, most of the time it's an application issue or a mixer applet issue. However, you haven't provided enough details to really debug this symptom. Start by providing a verbose PA log.

I've just explained above how the hardware is sometimes at fault and how sometimes the bugs need to be worked around (or the hardware plain disabled) in linux.
