Please start with a livecd, then run the command

ls -l /dev/disk/by-uuid >uuid.txt

Then attach the files uuid.txt, /media/xxx/etc/fstab and /media/xxx/boot/grub/menu.lst, with /media/xxx as the path where your disk is mounted.
