Further to my 2009-04-25 comment, no statistical picture has emerged. It is difficult to be sure of anything with such an intermittent problem but I have been getting very good results from cleaning the DVD since starting to use a micro-fibre cloth (sold for cleaning LCD screens) even when the DVD looks perfectly clean. If my impression (that slightly dirty DVDs can trigger the problem) is correct then it supports the idea that the cause of the problem is in error recovery.
