I can repro this with the latest Focal kernel (5.4.0-90) on:

    description: PowerNV
    product: 8247-22L (IBM Power System S822L)

Trying to see if I can repro it upstream.

FWIW my opening hypothesis is that something in a percpu data structure isn't getting updated over hotplug.
