To advise, nouveau is used for nvidia cards. Did you swap out/in a nvidia card at some point?

To clarify, you followed verbatim https://wiki.ubuntu.com/DebuggingKernelBoot#Capture_information_from_a_bad_boot_and_post_to_your_bug_report and while recording video with a cell phone or video recorder there is nothing on the screen from startup all the way to boot is frozen?

You may want to try using a live environment via http://cdimage.ubuntu.com/daily-live/current/ as this will provide a fresh boot versus one that may have upgrade cruft left over, and help rule some things out.
