I'm commenting here (and to 1394368) to conclude the discussion should anyone else be fighting with the same issue.
As Jeremy suggested I copied both these files from Windows' (Program Files\Common Files\QCA-Bluetooth) to Ubuntu's /lib/firmware/ar3k -folder:
- AthrBT_0x11020100.dfu
- ramps_0x11020100_40.dfu
Now there are no more errors on the dmesg and Bluetooth is working on both of these experimental kernels:
- 3.18.0-14-generic #15 SMP Fri Feb 27 18:38:10 UTC 2015 x86_64 x86_64 x86_64 GNU/Linux
- 4.0.0-040000rc1-generic #201502261652 SMP Thu Feb 26 16:56:29 UTC 2015 x86_64 x86_64 x86_64 GNU/Linux
Bluetooth still does not work on published kernels:
- 3.16.0-31-generic #41~lp1352821v1 SMP Fri Feb 20 20:52:34 UTC 2015 x86_64 x86_64 x86_64 GNU/Linux OR
- 3.16.0-30-generic #40-Ubuntu SMP Mon Jan 12 22:06:37 UTC 2015 x86_64 x86_64 x86_64 GNU/Linux
Even though Bluetooth now works, at least bluetooth mouse doesn't automatically reconnect after restart. Instead mouse text "flickers" in Bluetooth GUI screen. After clicking "ON" button (on bluetooth gui) for several time the bluetooth mouse might reconnect - or not.
