1) I would file a new report about that and provide the information following https://wiki.ubuntu.com/DebuggingKernelBoot .

2) Good to know it works in WinXP. Regarding .inf files, I just did a *.inf search (in Vista) and it would appear many are found in:
C:\Windows\System32\DriverStore\FileRepository

3) It would appear you may have not fully uninstalled, and reversed the previous steps you took when you were attempting an ndiswrapper configuration.

Testing ndiswrapper is requested largely to see if one would have a conceivable WORKAROUND and check for hardware error. Given you can't (easily) get it to work, and you tested WinXP, that would be sufficient for that part of things (although you may want to file a bug report against ndiswrapper if you tested it with the WinXP driver and it didn't work).
