I've upgraded to the 22 kernel in proposed and it seems to be working fine so far. I update if the problem occurs again.
@David Henningsson: There's been a ton of HDMI related regressions in Ubuntu 13.04 from 12.10. Do you know if this bug is related to HDMI volume not being saved and the inability to autoswitch from headphones to HDMI and vice versa?
