Bug Description

Under Quantal beta, my /var/log/syslog is flooded with warning messages. sylog is full of warning like this :
xhci_hcd 0000:00:14.0: WARN Event TRB for slot 1 ep 6 with no TDs queued?

(number after ep changes, but the rest is always the same). There is something like 10 such messages every second.

Would it be possible for you to test the latest upstream kernel? Refer to https://wiki.ubuntu.com/KernelMainlineBuilds . Please test the latest v3.6 kernel[0] (Not a kernel in the daily directory) and install both the linux-image and linux-image-extra .deb packages.

Once you've tested the upstream kernel, please remove the 'needs-upstream-testing' tag. Please only remove that one tag and leave the other tags. This can be done by clicking on the yellow pencil icon next to the tag located at the bottom of the bug description and deleting the 'needs-upstream-testing' text.

If this bug is fixed in the mainline kernel, please add the following tag 'kernel-fixed-upstream'.

If the mainline kernel does not fix this bug, please add the tag: 'kernel-bug-exists-upstream'.

If you are unable to test the mainline kernel, for example it will not boot, please add the tag: 'kernel-unable-to-test-upstream'.
Once testing of the upstream kernel is complete, please mark this bug as "Confirmed".

This issue appears to be an upstream bug, since you tested the latest upstream kernel. Would it be possible for you to open an upstream bug report[0]? That will allow the upstream Developers to examine the issue, and may provide a quicker resolution to the bug.

If you are comfortable with opening a bug upstream, It would be great if you can report back the upstream bug number in this bug report. That will allow us to link this bug to the upstream report.

Using latest upstream mainline kernel (3.7.0 rc1), the problem seems to be fixed.
I now only get a serie of warning when doing a lsusb -v
Otherwise, it seems to not happen (but maybe I'm not using the device that trigers the warning..)

Also, the SD card slot is not detected (no dmesg activity when card inserted or removed).

The rts5139 driver is responsible for both, the messages and also the non-working SD card slot.

After "sudo rmmod rts5139" the warnings that were printed every 5 seconds will stop.

It's still broken in
3.8.0-23-generic #34-Ubuntu SMP Wed May 29 20:22:58 UTC 2013 x86_64 x86_64 x86_64 GNU/Linux

There is currently no upstream fix available.

Regarding the problems described starting at comment 40 on 2013-03-27, with exception of comment 41,
I made fixes for them and submitted them upstream. Please consider backporting them to ubtuntu kernels.

franck, as per http://download.lenovo.com/express/ddfm.html an update is available for your BIOS (2.56). If you update to this following https://help.ubuntu.com/community/BiosUpdate , does it change anything?

If not, could you please both specify what happened, and provide the output of the following terminal command:
sudo dmidecode -s bios-version && sudo dmidecode -s bios-release-date

Please note your current BIOS is already in the Bug Description, so posting this on the old BIOS would not be helpful.

when enabling runtime PM of the usb3 controller, I start getting the same warning message[0] and the usb3 port become unusable (sometime i get an other error message too[2])
then when disabling the runtime PM of the controller and plugout/plugin the usb device, I don't get the warning message anymore
This happens on both my ubuntu & my archlinux (kernel-3.14.4) for the same reason
NOTE:
I'm also getting some other errors from time to time [2] [3]
My EFI/BIOS firmware is up-to-date but seems still buggy and I'm having some acpi compatibility issues, even I'm having bug on usb_wan module only on ubuntu caused mostly by the firmware [4]
