Web Cam No Longer works with 3.5.0-3-generic

My webcam stopped working with the 3.5 kernel series in Ubuntu. Here are my notes and errors messages with kernel versions:

after talking with apw on IRC he believes these error messages are related (I'm getting them on all 3.5 kernels)
