Bug Description

On my Lenovo T520 hot plugging USB3.0 PCI AKE Express USB 3.0 5Gbps BC618 Express card not possible. Hot plugging USB devices in this Card is also not possible. In kern.log:

Plugging out the USB3 Express card:
no messages in kern.log !!

WORKAROUND: Power on with PCI card and USB devices connected. Reconnecting devices after this is not possible. Re-Plugging the drive or other USB products are not recognized.

WORKAROUND: sudo echo acpiphp >> /etc/modules
Modify /etc/default/grub to have:
GRUB_CMDLINE_LINUX_DEFAULT="quiet splash pciehp_force=1 pciehp_poll_mode=1"

and run:
sudo update-grub

Roland, please be sure to confirm this issue exists with the latest development release of Ubuntu. ISO CD images are available from http://cdimage.ubuntu.com/daily/current/ . If the issue remains, please run the following command in the development release from a Terminal (Applications->Accessories->Terminal). It will automatically gather and attach updated debug information to this report.

Also, if you could test the latest upstream kernel available that would be great. It will allow additional upstream developers to examine the issue. Refer to https://wiki.ubuntu.com/KernelMainlineBuilds . Once you've tested the upstream kernel, please remove the 'needs-upstream-testing' tag. I'm pretty sure the latest nec/renesas firmware released in june/july 2012 fixes part of this bug. Since I upgraded the firmware of my NEC/Renesas cards, I can now observe hot-plugging functionality *only* for connected usb devices, but not for the usb3 expresscard. I did not test just before the firmware upgrade, so it's quite possible an ubuntu update has helped, but it's unlikely. I have yet to verify if the card still need to be inserted at boot time in order to be recognized, but for sure removing the card and reinserting it makes it not work afterward.
The first expresscard I have is a StarTech ECUSB3S254F which contains a Renesas uPD720200. I just upgraded its firmware to version 3.0.3.4. I had to do this under Windows 7.
The other expresscard is very similar to the above but without specific brand name. It contains a Renesas uPD720202(Revision 2). I just upgraded its firmware to version 2.0.1.3, also using Windows, but I think linux command line utility is available for this chip.

Did the firmware update to 4.0.2.0 for the uPD720200a card
(had to find a Windows note-book, while I'm running Ubuntu 12.04/64 on a Lenovo T520)

Problem is the same, Also see exactly the same behavior with a Dell/Windows7/64

disks and usb-sticks are only accessible, when booting the pc with USB30 card AND HD or stick plugged in.

if removing the disk/card or the USB30 card , the card isn't accessible.

strange thing, with lspci, the card is visible, although it's plugged out (and NO lines in the kernel logging)

Exact same problem.

Additional data point:
If I start with the card plugged in, insertion and removal of USB2.0 devices work fine.
If I insert a USB 3.0 device, nothing is visible in syslog.
If I insert the same USB 3.0 device in another USB2.0 port on the same machine, all works fine.
If I then try to insert a USB 2.0 device (that worked before) in the card, nothing visible in syslog anymore and drive is not accessible.
Stille works fine in another USB2.0 port
If I hot-plug or hot-remove the card, nothing is visible in syslog.

I'll try to perform some test later with another different machine and also check for firmware update and Windows behavior.

I'm not so sure anymore of the duplicate status.

Applying the workaround described in
https://bugs.launchpad.net/ubuntu/+source/udev/+bug/999462

solve the problem where the hot-plug/unplug of the card doesn't work.

Getting USB device to work reliably if not already plugged-in when you hot-plug the card doesn't work for me now.

this solves it, dit test it with a couple of devices:
- I can plug out and in the pci express card
- Adding usb sticks and hubs is working fine.
- plugging in a usb3 hard-disk into the card doesn't work, no messages in log-files
- plugging in the express-card together with a connected harddisk works !!
  (sometimes I have to retry it 1 or 2 times)
  when it fails I get: hub 4-0:1.0: unable to enumerate USB device on port 1
- plugging in firs a usb3 hub and later the hard-disk works

So it's not 100% solved, but I can work with this work-arround.

Please do not test the kernel in the mainline kernels archive directory daily folder. Once you've tested the upstream kernel, please comment on which kernel version specifically you tested. Roland, could you please test the latest upstream kernel available following https://wiki.ubuntu.com/KernelMainlineBuilds ? It will allow additional upstream developers to examine the issue. Please do not test the kernel in the mainline kernels archive directory daily folder, but the one all the way at the bottom. Once you've tested the upstream kernel, please comment on which kernel version specifically you tested. If this bug is fixed in the mainline kernel, please add the following tags:
kernel-fixed-upstream
kernel-fixed-upstream-VERSION-NUMBER

where VERSION-NUMBER is the version number of the kernel you tested. For example:

This kernel works partly !

After booting the system with card inserted and/or inserting the Card after it, the mouse response is extremely slow, after testing some times, this response problem is gone.

It works when card is inserted together with the USB device,
It works not when done seperately, see logging below:
(Tested this multiple times, always the same result)

Roland, as per http://support.lenovo.com/en_US/downloads/default.page?selector=expand an update is available for your BIOS (1.43). If you update to this, does it change anything?

If not, could you please provide the output of the following terminal command:
sudo dmidecode -s bios-version && sudo dmidecode -s bios-release-date

Regarding your post https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1006765/comments/43 , it appears you tested http://kernel.ubuntu.com/~kernel-ppa/mainline/v3.8.13.2-raring/ , which would not have been the latest mainline kernel available. Could you please test the latest via http://kernel.ubuntu.com/~kernel-ppa/mainline/v3.10-rc6-saucy/ and comment to the results?

@ Christopher, yes tried this, but 3.10 isn't running fine with 13.04 libs, modules are not probing, (including acpiphp ), so to test, I have to upgrade the complete system to a "non-stable" release.
Takes a little time, first have to make a disk-dump, for restore.

Ok, tried 3.10.0-031000rc6-generic #201306151835 SMP x86_64
with both 3.04 and 13.10

plug-and play isn't functioning at all, for the express card, do I have to configure it somewhere ?
it only works when booting with card and disk inserted.

hotplugging never woked for me out-of-the-box, with some acpiphp mods it was with kernel 12.04, but only when inserting card with disk connected to it.

Tested it with Ubuntu 3.9.0-6-generic #14-Ubuntu SMP x86_64
Some new behaviour

booting with express card inserted works, also inserting end reinserting the USB disk works fine.

after reinserting the card, it is detected again, but pluging in the USB disk, get this errors:
ACPI: Device does not support D3cold
(mouse behavour is slow again, till I removed the card)

Did as you asked, same result, booting with card and disk plugged in is fine, after that, nod disk and extreme slow mouse function,

Tried the latest 3.12.0-031200rc4-generic 64bit Kernel
This works much better:
- booting with card and USB-disk (USB-powered) inserted, is always working
- plugging in card first and after that a device, is not working
- plugging card with USB stick, work, but takes a long time
- plugging card with USB-disk (USB powered) together dousn work, and gives slow mouse response
- plugging card with USB-2-hub attached always works, it's possible to plug in/out disks and usb-sticks, but is slow
sorry I don't have a USB-3 hub, will buy one, if this works it's a very good work-around !!!

Julian Wiedmann, getting to https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1006765/comments/51 , correct me if I'm wrong here, but it seems the commit you documented wouldn't apply to this report, as it addresses a corner case when acpiphp is builtin statically, and a user wants the disable acpiphp, but cannot due to this static build in. The original reporter wouldn't want to disable acpiphp, as disabling it wouldn't appear to help this issue.

Roland, could you please undo any WORKAROUNDs and test the latest mainline kernel via http://kernel.ubuntu.com/~kernel-ppa/mainline/v3.12-rc7-saucy/ and advise to the results?

tried rc7, this is good!
removed all workarounds, but module acpiphp should be there ( of course )

after probing it, it works, but only when plugging in card, with disk plugged in together.
plugging first the card and after that the disk, or a usb stick isn't working, which is not big problem, if you know it.
plugging separately, which doesn't work, also gives the "slow-mouse-behavior"

with these syslog entry's

Roland, so just to confirm, with rc7 nothing changed in terms of what WORKAROUNDs work, and how it currently behaves, correct?

As well, could you please test the latest mainline kernel via http://kernel.ubuntu.com/~kernel-ppa/mainline/v3.12-saucy/ ?

Additional info:
what DOES works with 3.12.0-031200 :
plugging the card, together with USB2 hub works!
plugging the disk after that in the hub is also fine
I don't have a USB3 hub to test.
for all NOT working situations: the mouse behavior is slow, and hitching
