I confirm the bug with LibreOffice 4.0.1.2 (from ppa:libreoffice) on Ubuntu 12.04 Precise 64 bits.
Steps to reproduce are even more simple than what is described.
Steps to reproduce :
1 - open a new writer document
2 - write a bit of text and create a new ligne
3 - go to menu Insert > Objet > Formula
4 - Unselect the formula by two clics elsewhere on the text (no need to edit the formula)
5 - go to menu Insert : all menu lignes (or sub-menus) are grayed out except "Image" (this is normal behaviour if the formula was still selected)
How to have the menu again :
1 - save the document
2 - close it
3 - reopen it
(! once this did not work, I had to close and reopen the session)
How to "solve" the problem :
uninstall package libreoffice-gtk (and libreoffice-gnome ?), responsible for integrating libreoffice 4 into the global menu of Unity (libreoffice 4 does use package lo-menubar any more)
