/var/log/apt/term.log

which will hopefully have more information.

Also, please tell us how you initiated the upgrade. I'd recommend trying

sudo apt-get update && sudo apt-get -y dist-upgrade && sudo do-release-upgrade

and seeing whether that succeeds.
