I used Dan's patch (https://bugs.freedesktop.org/attachment.cgi?id=6762) on cairo-1.2.4, forced firefox to use the patched libcairo.so via LD_LIBRARY_PATH and now firefox starts up happily in 8bit/256color mode on a remote connection (X Server: Citrix ICA 256 color mode). I cannot test later versions of cairo unfortunately, but at least you have this proof that the patch addresses the "depth 8" mode. Keep up the good work!
