For other people who don't want to look it up, the new fixed icons are:
16x16/actions/bookmark-new.png
16x16/actions/dialog-apply.png
16x16/actions/folder-new.png

I'm re-opening because I can see a few more that need to be done:
dialog-error.png (as seen in Gaim, the 'Block' icon)
dialog-warning.png (as seen in Nautilus->Help->Report bug)
folder-saved-search.png (as seen in Liferea, the icon for 'Search Folders')

If I've missed any, someone please comment with them.

Also thanks a lot for working on this Kenneth, I've been wanting to see this fixed for quite a while
