Could you check the output of this grub-probe command (with three times 'v'):

    /usr/sbin/grub-probe --device-map="/boot/grub/device.map" --target=fs -vvv /boot/grub

If your LVM where /boot is instaled is large, it's possible that its bug (with a proposed patch) is your same case: https://bugs.launchpad.net/ubuntu/+source/grub2/+bug/1475337.
